Certified Occupational Therapy Assistant (COTA) – Skilled Nursing Facility (SNF) 

We are seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join a skilled nursing facility team providing short-term rehabilitation and long-term care services. This role offers the opportunity to work with a primarily geriatric population focused on restoring functional independence, improving safety, and enhancing quality of life. 

The COTA will implement individualized treatment plans developed by the supervising Occupational Therapist and play a key role in helping residents regain strength, mobility, and confidence following illness, surgery, or hospitalization. 

Key Responsibilities: 

  • Provide skilled occupational therapy interventions under OT supervision 

  • Assist residents with ADL retraining including dressing, bathing, grooming, toileting, and feeding 

  • Address functional mobility, balance, endurance, and safety awareness 

  • Implement fall prevention strategies and adaptive techniques 

  • Train patients in use of adaptive equipment and compensatory strategies 

  • Accurately document daily treatment sessions and progress in EMR 

  • Collaborate with interdisciplinary team including nursing, PT, SLP, and case management 

  • Maintain productivity standards while ensuring high-quality patient-centered care 

Ideal Candidate: 

  • Active COTA license in practicing state 

  • SNF or geriatric experience preferred 

  • Strong time management and clinical reasoning skills 

  • Comfortable managing a varied caseload of short-term rehab and long-term residents
