We're hiring a Certified Occupational Therapy Assistant for a travel contract role at a Skilled Nursing Facility in Libertyville, IL. Minimum Requirements

  • Associate degree in Occupational Therapy from an accredited college.
  • Active Illinois COTA license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience working in skilled nursing facility or long-term care settings preferred.
  • Ability to document treatment plans and progress notes per facility and state regulations.
  • Experience with ADL retraining, adaptive equipment recommendations, and fall prevention programs preferred.


Assignment Details

  • Facility Type: Skilled Nursing Facility.
  • Shift Name: Days.
  • Shift Details: Days 08:30–16:30.
  • Schedule: Five 8-hour shifts per week.
  • Floating: Candidate may be required to float to other facilities as needed.
