Job Title: Transitions of Care Coordinator

Location: New York, NY 10004

Duration: 2 months

Pay Rate : 35$ - 40$ / Hr on W2

Shift: Monday to Friday 9 AM to 5 PM

· The Transitions of Care Coordinator will fulfil the critical Medicare MOC requirements related to sharing with admitting facilities a member’s plan of care as well as informing the PCP of record of their patient’s admission and discharge from the acute care setting, skilled nursing facilities and home care.

Required Education, Training & Professional Experience

  • Bachelor’s degree required.
  • Minimum 2 years of work experience in healthcare and customer service.
  • Proficiency with computers navigating in multiple systems and web-based applications.
  • Ability to prioritize and manage changing priorities under pressure.
  • Must know how to use Microsoft Office applications including Word, Excel, PowerPoint, and Outlook.

Product of Role & Responsibilities

  • Ensure current plans of care are sent to acute care facilities for admitted members.
  • Ensure PCPs are notified of admissions to acute care facilities and discharge dates.
  • Track POCs sent and received.
  • Timely data entry into Disease Care Management System (DCMS).
  • Collaborate with the Business Analyst to generate reports POCs mailed to members.
  • Demonstrate excellent customer service when speaking with facility staff and providers.
  • Perform other duties deemed necessary to carry out functions and processes within the employee’s capacity in the Department
