Givens Estates Health Center is looking to hire a full-time Transitions Coordinator to support our Skilled Nursing team. The primary purpose of this position is to work under the direction of a RN in implementing plan of care for the residents, being responsible for the Restorative Program. This position acts as a liaison between formalized therapy and nursing to promote restorative nursing philosophy among all facility departments. 

What you'll do:

  • Provide direction and leadership through delegation of assignments to Restorative team
  • Partner with the qualified therapists to ensure specific Restorative Program recommendations are followed through when a resident is discharged from skilled therapy 
  • Participates in identification of residents with restorative needs, collaborates with the care planning team and therapy team to establish goals and interventions to meet the residents' restorative needs
  • Participate in the supervision of care provided to residents by RNs/CNAs including preventative measure for skin integrity
  • Participate in Rehab/Restorative meetings to discuss each resident's restorative program from progress or lack thereof and make changes as needed
  • Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor 

What you'll need:

  • 1 year minimum experience as an LPN in long term care setting preferred
  • Must have unencumbered nursing license
  • Ability to seek out new methods and principles and be willing to incorporate them into existing practices
