Summary:
- Location: In-person visits in Northeast Ohio; Cleveland, Akron, Canton, Ohio
- Work Mode: Hybrid (In-person and remote)
- Shift: Mon-Fri 8a-5pm EST; 30 min lunch
- Duration: 6 months with potential to extend based on need
Responsibilities:
- Assist members in Northeast Ohio with SSDI applications through in-person interactions in SNFs
- Gather information via interviewing, obtaining signatures, and visiting local SSA offices
- Launch the expansion of SOAR across the state
- Review SNF census, navigate internal systems, and gather medical records following detailed SOP for SOAR SSA
- Communicate effectively with members, including those with language and speech difficulties
- Submit required paperwork and follow up timely according to feedback
Requirements:
- Bachelor's degree or equivalent experience
- 2-4 years of related experience in social work, nursing, or skilled long-term care facility
- Experience with documentation and member-facing communication skills
- Organized, detail-oriented, and task-focused
Preferred Skills:
- SOAR certification
- Experience with SOAR applications
- ADL (activities of daily living) and EHR clinical knowledge
- Ability to write reports and complete documentation in detail
- Independent work and troubleshooting skills
- Effective communication with diverse populations
