What Youll Do:
- Conduct assessments and develop treatment plans for residents requiring speech and language therapy
- Provide therapy to improve communication, swallowing, and cognitive-communication skills
- Participate in interdisciplinary team meetings to coordinate patient care
- Document clinical notes and progress in compliance with facility and regulatory standards
- Take on some Director of Rehabilitation (DOR) responsibilities, including scheduling and attending meetings
- Support compliance with PDPM processes and facility regulations
- Lead and collaborate with the rehab team (PT, OT, ST, PTA, COTA) to optimize patient outcomes
What You Need:
- Valid state license/certification as an SLP
- Minimum of 1 year relevant experience, preferably in LTC or skilled nursing settings
- Strong leadership, communication, and team collaboration skills
- Ability to work independently and adhere to regulatory standards
