What You’ll Do:
  • Conduct assessments and develop treatment plans for residents requiring speech and language therapy
  • Provide therapy to improve communication, swallowing, and cognitive-communication skills
  • Participate in interdisciplinary team meetings to coordinate patient care
  • Document clinical notes and progress in compliance with facility and regulatory standards
  • Take on some Director of Rehabilitation (DOR) responsibilities, including scheduling and attending meetings
  • Support compliance with PDPM processes and facility regulations
  • Lead and collaborate with the rehab team (PT, OT, ST, PTA, COTA) to optimize patient outcomes

What You Need:
  • Valid state license/certification as an SLP
  • Minimum of 1 year relevant experience, preferably in LTC or skilled nursing settings
  • Strong leadership, communication, and team collaboration skills
  • Ability to work independently and adhere to regulatory standards
