What You’ll Bring to the Role:
  • Proven RN clinical skills with a strong foundation in patient care
  • Ability to assess patient needs and develop customized care plans
  • Excellent communication skills to collaborate effectively with patients, families, and multidisciplinary teams
  • Critical thinking and problem-solving capabilities, especially for complex cases
  • Comfort working independently in a home health setting

Nice to Have Skills:
  • Experience with chronic pain management, substance abuse, or care coordination
  • Knowledge of emergency department procedures and frequent utilizers management
  • Familiarity with healthcare regulatory standards and documentation

Preferred Education and Experience:
  • Valid Hawaii RN license and current CPR certification
  • At least 2 years of home health or community nursing experience preferred
  • Strong understanding of case management and discharge planning

Additional Requirements:
  • Must be willing to travel within the K?ne‘ohe area to patient homes
  • Certification in case management or related specialties is a plus
  • Ability to work weekdays during daytime hours
