What Youll Do:
- Conduct patient screenings and evaluations for cognitive and ADL physical functioning disorders.
- Develop individualized care plans compliant with regulatory and clinical standards.
- Administer therapeutic interventions, including ADL retraining, exercises, modalities, and use of support devices.
- Monitor patient progress and modify treatments based on outcomes.
- Provide patient, family, and caregiver education and counseling.
- Document all assessments, treatments, and discharge summaries thoroughly in electronic health records.
- Collaborate with healthcare team members to support patient rehabilitation goals.
- Support supervision of assistants, rehabilitation aides, or students as needed.
What You Need:
- Valid OT license and NBCOT certification.
- Prior experience with adult or long-term care populations preferred.
- Strong clinical assessment and intervention skills.
- Ability to communicate effectively with team members and patients.
