What Youll Do
- Observe residents continuously, noting changes in mental/physical well-being and escalating concerns to the appropriate supervisor/physician.
- Receive and carry out physician orders accurately, including medication administration and required documentation in MARs/TARs and electronic records.
- Administer medications and treatments per order, including documentation and maintenance of unit pharmaceuticals.
- Supervise LNA staff in accordance with licensing requirements, support resident care assignments, and assist with nursing care as needed.
- Support clinical coordination by accompanying physicians on rounds and communicating resident status during shift handoff and review processes.
- Provide care including oxygen administration per facility procedures; administer gastrostomy/jejunostomy feedings per policy; perform wound care and have knowledge of wound vacs.
- Handle resident care operations such as admissions/discharges/transfers documentation, unit reporting, new orders review, and compliance with controlled medication storage/count procedures.
What You Need
Required
- Active LVN/LPN license (per state requirements).
- Ability to administer medications and treatments and complete accurate electronic documentation (MARs/TARs).
- Experience providing long-term care nursing support (specific years not specified).
Preferred
- Familiarity with supervising and coordinating care with nursing assistants (LNA supervision experience preferred).
- Experience with wound care (including wound vac knowledge) and oxygen/g-tube/j-tube care is a plus (not required).
