Job Title: Temp - Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Long Term Care (LTC)

Job Description:

This assignment location is 2 hours from Boston, MA! Please note, candidates MUST have their own transportation as this facility is located in a very rural area with limited housing options and public transportation. Please ensure your traveler is aware and is looking into options at the time of submission.

• Observe each resident on a continuing basis, alert to changes in mental and physical well-being; notify the Supervisor of pertinent observations and discuss the need to notify the physician; receive Resident Status Reports from off-going shift and give detailed reports to the incoming shift of each resident's status and events.
• Accurately receive, transcribe, and carry out physicians’ orders.
• Administer medications and treatments as ordered by the physician and properly document via a computerized system of MARs/TARs and electronic documentation. Order and maintain all pharmaceuticals for the units.
• Supervise LNAs in accordance with licensing requirements under the New Hampshire Board of Nursing. Assign and measure nursing staff resident care, supervise the care given, and assist direct-care staff with nursing care as necessary.
• Accompany the physician on resident rounds, address residents' medical needs, and facilitate communication during 60-day reviews.
• Administer oxygen in accordance with facility guidelines and procedures. Perform gastrostomy/jejunostomy feedings, wound care, and have knowledge of wound vacs.
• Perform routine duties such as checking appointments, therapy schedules, activity boards, and documenting nutritional charts. Monitor LNA documentation and infection control practices, ensuring the unit is clean and hazard-free.
• Admit, discharge, and transfer residents, completing documentation and notifications as necessary.
• Complete 24-hour reports, check new orders, and communicate resident issues or concerns with staff and management.
• Communicate with residents' families to resolve concerns, notify family members of orders, and obtain consent when necessary.
• Handle incoming calls appropriately.
• Manage assigned units during shifts under the supervision of the Nurse Manager or Nursing Supervisor.
• Follow regulations regarding storage, usage, and documentation of controlled medications, including shift change counts. Report discrepancies.
• Assist with orientation and ongoing education of nursing staff, evaluate staff performance, and recommend disciplinary actions when necessary.

Hourly Pay Rate: $40.87
