Clinical Opportunities

Clinical Support Coordinator - Laurel Fork, VA

Tri-Area Community Health is seeking a full-time Clinical Support Coordinator to oversee medical clinical support programs and lead our clinical teams. As a key member of the Middle Management Team, you will ensure efficient service delivery across all sites by collaborating with the Medical Director and department heads.

Core Responsibilities

  • Leadership: Supervise Clinical Site Managers, Care Coordination, Health Education, and Dietary services.

  • Program Oversight: Manage Diabetes Education, Food and Nutrition, and Mobile Unit operations.

  • Operations: Develop clinical policies, manage medical equipment procurement, and assist with grant preparation and compliance.

  • Quality Care: Participate in Quality Improvement programs and resolve complex patient complaints.

  • Clinical Support: Provide direct clinical services during vaccination events or at specific sites as needed.

Requirements

  • Licensure: Current Virginia Registered Nurse (RN) license.
  • Education: BSN preferred.
  • Experience: Previous healthcare management experience is required; four years and FQHC experience preferred.
  • Skills: Strong background in nursing principles, BLS certification, and a willingness to mentor staff.
  • Logistics: Ability to pass a background check and maintain all licensure requirements.

School-Based Medical Provider - Patrick County, VA

Tri-Area Community Health (https://triareahealth.org/) is a Federally Qualified Health Center headquartered in Laurel Fork, Virginia, with full-service locations currently open in Ferrum, Floyd, Fries, Cana, Carroll County Schools, Grayson Highlands (Troutdale), Galax City Schools, Grayson County Schools, and Laurel Fork, Virginia. For more than 40 years, Tri-Area has provided care to underserved individuals in Southwest Virginia. We offer integrated primary medical and behavioral health care in all these clinic locations. We also have pharmacies that are open in Ferrum, Fries and Laurel Fork. Our three large sites are National Committee for Quality Assurance-certified Patient-Centered Medical Homes and medical professionals at all sites are eligible for National Health Service Corps Loan Repayment (HPSA score of 19).We are excited to be opening a new school-based clinic in collaboration with Patrick County Public Schools.

POSITION:We are seeking a Family Nurse Practitioner (autonomous preferred) or Physician Assistant to work year-round, 5 days a week (Monday through Friday from 7:30 am - 4:00 pm) in our new Patrick County Public Schools Clinic. This position involves providing direct medical care of elementary, middle, and high school students and School System staff. The work is performed independently with oversight provided by the Medical Director. The candidate must possess the ability to provide acute care for a variety of needs and be competent in performing related tasks such as minor suturing. The Medical Provider must operate in accordance with the terms and conditions of the various contracts and grants providing the funding for Tri-Area's programs and projects. "Tri-Area Community Health clinical staff may qualify for National Health Service Corp loan repayment through https://nhsc.hrsa.gov/

REQUIREMENTS: Nurse Practitioners must be licensed to practice as a Family Nurse Practitioner in the Commonwealth of Virginia (autonomous strongly preferred). Physician Assistants must also be licensed to practice in the Commonwealth of Virginia.

School-Based IT Support Technician - Laurel Fork, VA

Tri-Area Community Health (https://triareahealth.org/) is a Federally Qualified Health Center headquartered in Laurel Fork, Virginia, with locations currently open in Ferrum, Floyd, Fries, Cana, Carroll County Schools, Grayson Highlands (Troutdale), Galax City Schools, Grayson County Schools, and Laurel Fork, Virginia, with expansion into Floyd County Schools and Patrick County Schools in the fall of 2026. Tri-Area Community Health is hiring a full-time School-Based IT Support Technician to support all school-based clinics in our coverage area. The School-Based IT Support Technician provides on-site and remote technology support for school-based health clinic locations across multiple school systems (Carroll County, Grayson County, Galax City, Floyd County and Patrick County). This position focuses on maintaining reliable operation of telehealth equipment, computers, printers, communication devices, and other technology used by clinical staff operating within school environments. This role requires frequent travel to school sites to install, maintain, and troubleshoot equipment supporting telehealth visits, electronic health record access, and day-to-day clinic operations. The technician works independently in the field while coordinating with the central IT department to ensure secure, reliable, and efficient technology services for school-based healthcare programs. Hours are primarily M-F, 8-5 with a potential for an alternate schedule such as 4 10-hour days.

This position requires a high school diploma or equivalent; an associate degree, technical certification, or equivalent experience in Information Technology is preferred.

Knowledge of current computer hardware (laptops, desktops, tablets, printers, scanners, telehealth devices, and other peripheral equipment) and common software applications used in healthcare and school environments is required.

Strong interpersonal and communication skills are essential to effectively assist school personnel and clinical staff with varying levels of technical knowledge.

Telemental Behavioral Health Educator and Outreach - Floyd, VA

Tri-Area is seeking a full-time Telemental Behavioral Health Educator and Outreach Worker for our Floyd County Public Schools clinic. This is a full-time, year-round position (40 hours per week), which may include occasional evenings and weekends. Although some work may occur in Tri-Area Community Health clinics, most of the duties for this position will be performed in the schools and community. The coverage area includes all Floyd County Public Schools, serving students who attend those schools, their families, and employees of the school division, in partnership with a co-educator, and daily travel is anticipated (mileage will be reimbursed).

This position is primarily responsible for working with students, families and employees of Floyd County Public Schools, as well as interacting with community members and organizations regarding the behavioral health of youth. This person will provide telehealth assistance and training to school employees, health education to students and family members, and outreach to the community.

  • An associate degree in human services is required for this position with preference given to those holding a bachelor's degree in human services or education.
  • Two years of work experience in education, health care, or human services is required.
  • Knowledge of behavioral health, positive attitude toward persons with behavioral health concerns, and willingness to work with youth who have behavioral health issues is required.
  • Excellent interpersonal skills, including the ability to interact professionally and gain trust with culturally diverse individuals, are required.
  • Ability to handle confidential information with discretion and professionalism is required.
  • Computer skills are required.
  • Access to reliable transportation is required.
  • Successful completion of a satisfactory background check is required.
  • Prior experience in a health educator and/or in an outreach worker or similar role is strongly preferred.
  • Familiarity with the service area is strongly preferred.
