As an ACT (Assertive Community Treatment Team) Team Leader, you will lead a multidisciplinary team dedicated to supporting individuals living with serious mental illness as they build stability, independence, and meaningful lives in the communities they call home.
This is more than a leadership role. It is an opportunity to guide clinicians, nurses, peers, and community support specialists in delivering evidence-based, person-centered care to individuals who are often navigating some of life's most complex challenges.
