• Credential Copy (copy must show document number and current expiration date)
• Letter of Introduction
• Letter(s) of Reference (Three recent letters with original signatures)
• Resume
• Education Specialist Instruction Credential - Mild/Moderate (or MMSN authorization ) OR Education Specialist Instruction Credential - Moderate/Severe (or ESN authorization)
