Position Summary
The Targeted Case Manager (BCM Level) provides targeted case management services to adult residents of Philadelphia who are recovering from severe mental illness and may have co-occurring substance use disorders and/or experiences with homelessness.

Key responsibilities include:

  • Linking participants to appropriate services and resources
  • Monitoring service delivery and participant progress
  • Assisting individuals in gaining access to needed care
  • Conducting assessments and developing service plans
  • Addressing and resolving barriers to care
  • Building support networks
  • Utilizing community resources to support recovery

This role focuses on empowering individuals by leveraging their strengths and supporting them in achieving the highest level of independent functioning within the community.

Work Environment

  • Approximately 75% of time is spent in community-based settings and outreach
  • Travel is required and may include walking, driving agency vehicles, or using public transportation based on program needs

Minimum Requirements

    • Associate’s Degree in:
      • Sociology
      • Social Welfare
      • Psychology
      • Gerontology
      • Other social science fields
    • + 3 yearsof mental health direct care experience

    Or

    • Bachelor’s Degree in:
      • Sociology
      • Social Work
      • Psychology
      • Gerontology
      • Anthropology
      • Political Science
      • History
      • Criminal Justice
      • Theology
      • Counseling
      • Education
      • Other related social sciences
    • + 2 yearsof mental health direct care experience
