Provides direct supervision and operational oversight of Population Health Case Managers and Lead Population Health Case Managers. Responsible for achieving program goals through coordination of care management activities, staff performance management, and workflow execution across population health initiatives. Supports the delivery of care coordination services that improve patient outcomes, reduce avoidable utilization, and address social drivers of health (SDOH).
Leads day-to-day operations of care management programs including transitions of care, chronic disease management, Interactive Home Monitoring (IHM), Remote Patient Monitoring (RPM), and Remote Therapeutic Monitoring (RTM). Ensures staff adherence to established workflows, documentation standards, and regulatory requirements. Partners with leadership and interdisciplinary teams to support quality improvement, health equity initiatives, and value-based care performance.
MINIMUM REQUIREMENTS
Speech Language Pathology -
Education: Master's Degree in Speech-Language Pathology.
Experience: Must have acute care hospital experience. Prefer 3 years of demonstrated leadership experience
Licensure: Licensed to practice Speech-Language Pathology from the Virginia Board of Examiners for Audiology and Speech Pathology required. Eligible for Clinical Fellowship Year (CFY) or certified with Certificate of Clinical Competence (CCC) is required. American Heart Association (AHA) Health Care Provider BLS certification required.
Physical Therapy-
Education: Minimum-Bachelor's Degree in Physical Therapy Prefer master's or Doctoral Degree
Experience: Must have acute care hospital experience. Prefer 3 years of demonstrated leadership experience
Licensure: Valid license or registration to practice in the Commonwealth of Virginia in Therapy discipline required. American Heart Association (AHA) Health Care Provider BLS certification required.
Occupational Therapy -
Education: Minimum-Bachelor's Degree in Occupational Therapy Prefer Master's or Doctoral Degree
Experience: Must have acute care hospital experience. Prefer 3 years of demonstrated leadership experience
Licensure: Valid license or registration to practice in the Commonwealth of Virginia in Therapy discipline required. American Heart Association (AHA) Health Care Provider BLS certification required.
PHYSICAL DEMANDS
Job requires standing for prolonged periods and frequently bending/stooping/traveling. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to blood/body fluids and infectious disease.
The starting base rate for this role is $76,420.00 annually. Individual compensation will be determined by the selected candidate"s qualifications, previous work experience, and/or education.
