About the Role

The Supervisor of Clinical Services/Clinical Manager ensures that patient care is delivered with the highest standards of quality, efficiency, and compassion. This position provides leadership and oversight to patient care staff, while supporting day-to-day clinical operations.

You will provide oversite and responsibility of all clinical activities, playing a vital role in staff development, care coordination, and compliance with home health regulations.

Primary Responsibilities

  • Lead, guide, and support the patient care team to deliver exceptional home health services
  • Manage day-to-day clinical operations including triage, visit scheduling, and field staff support
  • Assesses patient and family needs through record review, case conferencing and interdisciplinary meetings and develops a plan to meet those needs directly or as delegated to others.
  • Support high-quality documentation and OASIS accuracy through education, review, and collaboration with the OASIS Specialist
  • Ensure compliance with policies, procedures, and regulatory guidelines
  • Oversee staff training, competency requirements, new staff orientation, and ongoing professional development
  • Directly supervise patient care staff: monitor productivity, conduct on-site visits, assist with care and discharge planning, complete annual reviews, manage PTO, and oversee compliance for LPN supervision and disciplinary processes
  • Ensure proper staffing through referral and caseload management; communicate updates to interdisciplinary teams and the Intake Department
  • Coordinate on-call schedules and participate in supervisor on-call rotation
  • Foster team morale and promote Stratford VNA's Mission, Vision, and Core Values
  • Participate in Performance Improvement initiatives and quality‑enhancement activities
  • Lead staff meetings and educational sessions for clinical and documentation standards

Qualifications

  • Graduate of an accredited school of nursing; current Connecticut RN license required
  • Bachelor's degree in nursing preferred, or completion of required community health nursing courses per CT DPH guidelines
  • Minimum two years of RN home health experience required
  • Prior management or supervisory experience strongly preferred
  • Solid understanding of OASIS and home health regulations
  • Strong clinical judgment, critical thinking, and problem‑solving skills
  • Excellent communication, presentation, and team leadership abilities
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Commitment to cost‑effective, high‑quality patient care
  • Valid driver's license and current auto insurance required
