A completed application includes: • Employment application for substitute Nurse • Verification of Education (official copy of a public high school transcript or college transcript) • DPS CCH Verification Form • Criminal History Record Information Form (CHRI) • W-4 form • Employment Eligibility Form or I-9 (including a copy of your social security card and drivers license) • Acceptable User Policy Agreement (AUP) ALL SUBSTITUTE APPLICANTS MUST BE FINGERPRINTED • The fingerprinting process will be initiated once your application is received.
