Position Summary
The Middle/High School Student Support Coordinator (SSC) serves as a critical member of the Student Services team, working to ensure that all students—particularly those identified as at-risk—receive the academic, social-emotional, and community-based supports necessary to succeed. This role is responsible for executing the school’s support plan, strengthening school-community partnerships, and aligning services to meet the needs of the student population in accordance with school leadership priorities.
The SSC collaborates closely with school administrators, teachers, and student support personnel to remove barriers to learning and foster a positive, inclusive school culture.

Essential Duties and Responsibilities
Student Support & Case Management
  • Collaborate with the Student Services team and school leadership to identify at-risk students in need of support.
  • Case manage approximately 10% of the student population, including monitoring progress and providing individualized support.
  • Develop, implement, and monitor targeted intervention plans.
  • Coordinate supports to address risk factors that may contribute to disengagement or dropout.
Program Implementation & Coordination
  • Execute the school-wide Student Support Plan with fidelity.
  • Coordinate schedules and services for internal staff and external service providers.
  • Monitor, adjust, and report on the effectiveness of supports and interventions.
  • Maintain accurate documentation and data tracking systems related to student services.
Community Partnerships & Resource Development
  • Build and sustain partnerships with community organizations, agencies, and service providers.
  • Serve as the site coordinator for community-based programs and partnerships.
  • Facilitate access to resources that support students’ academic, behavioral, and social-emotional needs.
School Climate & Culture
  • Support initiatives that positively impact school-wide climate and culture.
  • Collaborate with staff to implement strategies that promote student engagement, belonging, and well-being.
  • Participate in school-based teams focused on attendance, behavior, and student success.
Collaboration & Communication
  • Work closely with administrators, teachers, counselors, and other support staff to align services and supports.
  • Communicate effectively with students, families, and stakeholders regarding supports and student progress.
  • Participate in meetings, data reviews, and professional development opportunities.

Required Experience:
Qualifications
  • Bachelor’s degree in Education, Human Services, Social Work, Community Outreach, or a related field required. Minimum of 3–5 years of experience working with youth, families, or community-based programs, preferably in a school or nonprofit setting.
  • Strong organizational, communication, and leadership skills.
  • Ability to manage multiple schedules, priorities, and deadlines effectively.
  • Ability to build and maintain positive relationships with students, families, staff, and community partners.
  • CPR/First Aid certification preferred or willingness to obtain.
  • Proficiency in Microsoft Office and/or Google Workspace.
