Job Summary and Qualifications
The stroke coordinator is a health care professional who works in collaboration with the medical and hospital staff to coordinate multidisciplinary care to the neuroscience diagnosed patient. The coordinator provides clinical, professional expertise for patients, families, and staff members. Scope of practice includes providing organized, evidence based care to the patient with neuroscience diagnosis focusing on health promotion and disease prevention. The coordinator also functions as an educator and coordinates neuroscience diagnosis care management to promote optimal patient outcomes. The coordinator will collect and analyze data related to stroke, assist with documentation, coding, clinical care and outcomes. The ability to retrieve, communicate or otherwise present information in a written, electronic, auditory or visual fashion is essential. The Stroke Coordinator attends and conducts Stroke Program meetings and is responsible for the evaluation of outcomes and policy.
