Social Worker – Short Term Residential (SRT)


Position Summary
South County Mental Health Center, Inc. is seeking a compassionate and dedicated Social Worker to join our Short Term Residential (SRT) program. This position provides assessment, discharge planning, case management, and community linkage services for adults receiving residential behavioral health treatment. The Social Worker collaborates with clients, families, and interdisciplinary team members to support stabilization, recovery, and successful transition back into the community.

Minimum Qualifications
  • Bachelor’s Degree in Psychology, Social Work, or a related field from an accredited college or university
  • Minimum one (1) year of experience working with chronically mentally ill, dually diagnosed, or emotionally disturbed adults
  • Strong clinical assessment and case management skills
  • Working knowledge of behavioral health documentation and treatment planning
  • Proficiency with computers and electronic documentation systems


Preferred Qualifications:
  • Two (2) or more years of experience working with chronically mentally ill, dually diagnosed, or emotionally disturbed adults
  • Experience in residential, inpatient, or behavioral health treatment settings
  • Knowledge of community resources, housing services, and discharge planning processes


Duties and Responsibilities
Client Care & Treatment Planning
  • Evaluate client and family needs and identify appropriate services and supports
  • Participate in weekly multidisciplinary treatment team meetings
  • Assist in developing and coordinating individualized treatment goals
  • Conduct weekly discussion groups and therapeutic support activities for clients
  • Support clients throughout the residential treatment process
Discharge Planning & Community Linkage
  • Develop comprehensive discharge plans and recommendations
  • Link clients with appropriate community resources and support services
  • Coordinate discharge planning with nursing, medical providers, and residential staff
  • Develop discharge goals and document progress toward successful community transition
Documentation & Record Keeping          
  • Develop comprehensive discharge plans and recommendations
  • Link clients with appropriate community resources and support services
  • Coordinate discharge planning with nursing, medical providers, and residential staff
  • Develop discharge goals and document progress toward successful community transition
Training & Professional Development
  • Develop comprehensive discharge plans and recommendations
