Position Summary

We are looking for a Senior Quality Improvement Analyst with a strong interest in population health and experience supporting quality and performance measures tied to contracts. The ideal candidate will be organized, collaborative, and skilled in translating complex data into clear actionable insights that drive improvements in quality and contract performance across pediatric care. This role partners closely with clinical and operational teams to monitor, report on, and improve population-based measures such as well-child visits and immunization status. Strong presentation and project management skills, along with a team-oriented mindset, are essential to advancing impactful measure-driven quality initiatives across our network.

The Senior Quality Improvement Analyst will play a strategic role in advancing a culture of quality and continuous improvement within PCCN and the practices affiliated with its clinically integrated network. As a senior member of the PCCN Quality Team, this role will be responsible for leading and overseeing quality improvement initiatives with a focus on quality performance tracking, reporting, and achieving measurable outcomes. The Senior Quality Improvement Analyst will work closely with internal and external stakeholders, including payor partners, to drive initiatives that improve patient care, address healthcare disparities, and enhance the performance of the network in value-based contracts.

Key Responsibilities

• Develop and implement data-driven strategies to identify and close gaps in care • Track quality performance metrics • Generate reports that provide actionable insights for the networks stakeholders • Manage multiple high-impact quality projects simultaneously, ensuring alignment with organizational goals and driving continuous improvement efforts • Collaborate with cross-functional teams to analyze performance data, identify opportunities for improvement, and develop innovative solutions to enhance patient care

Position Duties

Quality Improvement

  1. Assists in designing, implementing, and maintaining a comprehensive continuous quality improvement (QI) program.
  2. Works with PCCN leadership and staff to develop and integrate quality improvement processes into all aspects of PCCNs clinical programs and operations.
  3. Serves as a subject matter expert on NCQAHEDIS and CMS Core quality measures, as well as AHCCCS quality programs for PCCN and its practices.
  4. Contributes to a culture of continuous improvement and excellence, promoting standardization, efficiency, efficacy, and safety across the institute.
  5. Analyzes clinical information from various internal and external sources relating to patient safety, utilization of resources, system problems, and other quality functions.
  6. Demonstrates strong project management skills by organizing and overseeing multiple quality improvement projects concurrently.
  7. Stays up to date on industry trends, regulations, and best practices related to quality performance and value-based care.

Network Quality Interventions

  1. Reviews practice performance data for trends in the context of best clinical practice and contractual quality goals.
  2. Works with PCCN leadership and staff to implement a comprehensive performance improvement program for PCCN and its practices using formal process improvement techniques, including but not limited to PDSA DMAIC and Lean Six Sigma.
  3. Uses performance trends to develop plans for practice engagement, education, and process improvement.
  4. In coordination with the Practice Integration team, provides ongoing process improvement support to PCCNs practices by frequently reviewing practice-level performance and outcomes data, observing clinical workflows, identifying root causes, implementing ongoing performance improvement activities, and evaluating the effectiveness of performance improvement activities on an ongoing basis.
  5. Contributes to the development and maintenance of a library of quality performance best practices for PCCNs practices.
  6. Contributes to the development and implementation of innovative approaches to address patient gaps in care specific to PCCNs contractual quality measures.
  7. Works collaboratively with network practices in obtaining patient records and supplemental data to submit to partnered health plans.
  8. Communicates clearly and effectively with internal teams, external stakeholders, and payor partners.
  9. Develops positive relationships with key stakeholders to facilitate collaboration and information exchange.
  10. Collaborates with payor partners to plan Clinical Quality Meetings, including agenda development and scheduling.

Quality Performance Reporting

  1. Presents PCCNs contract performance and population health initiatives in meetings with internal and external stakeholders.
  2. Generates reports and presentations to communicate progress and outcomes to stakeholders.
  3. Reports data from quality initiatives to appropriate individuals, departments, and committees through interpretive presentations, automated electronic reports, and/or publications.
  4. Assists in the development and maintenance of performance dashboards indicating performance against contractual obligations and/or population health initiatives in coordination with the data and analytics team.
  5. Works with Phoenix Childrens research teams to support PCCNs quality initiatives in the development of research papers and academic writings.

Performs miscellaneous job-related duties as requested.
