Overview:
The Sr. DSP Medical Coordinator is responsible for all of the duties of a Direct Support Professional plus scheduling, tracking and documenting all medical appointments, as well as scheduling all labs and required tests.

Responsibilities:
Document all paperwork required, including incident reports, log entries and Res. Hab. progress notes when appropriate
Track all medical appointments and follow-up appointments and document in tracking forms
Track all lab tests, as well as other tests and ensure correct documentation in medical tracking log
Document all consults, refusals and MD recommendations appropriately
Ensure that medication inventories are completed at least weekly, ad will order new medication as prescribed, in a timely manner, and maintain the medication storage area in accordance with regulations
Identify medication errors and communicate the errors to the nurse and manager effectively
Communicate with the nurse and manager via a Medical Communication Log
Follow all AMAP procedures
Attend ISP or other meetings when requested
Maintain open lines of communication with management and staff
Qualifications:
NYC Driver license or driver license required
HS Diploma or GED required
1+ year of HHA experience required, CNA experience a plus
2+ years of Direct Support Professional experience a plus
