Serve as a Resource & Liaison - Build relationships with community clergy and connect patients with spiritual care services as needed. Participate in Interdisciplinary Team Meetings - Collaborate with hospice staff to integrate spiritual care into patient-centered care plans. Offer Crisis Intervention & Bereavement Support - Assist families and caregivers through grief, trauma, and stress, providing compassionate guidance during difficult times. Document & Maintain Patient Care Plans - Ensure accurate and timely documentation of patient needs, interventions, and family support efforts Other Duties as Assigned - The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
