In this role, you will perform comprehensive evaluations, develop and implement individualized treatment plans, document outcomes, and collaborate closely with physicians, nurses, and rehab teams to ensure patient-first, evidence-based care. You’ll educate patients and families, participate in interdisciplinary case conferences, and contribute to quality improvement.
