Responsibilities
This is a WOW (Work on Weekends) position. Schedules for WOW employees include every weekend (Nights - Friday & Saturday, Days - Saturday & Sunday). Employees in designated WOW positions earn premium pay in addition to their competitive hourly rates and shift differential.
The Social Worker is a member of the multidisciplinary team. The Social Worker collaborates directly with the care team, care management team and patient, and/or family, and/or the patient's representative to assess the patient's continuum of care needs and ensure the patients transition to the least restrictive care environment. As an independent professional, the Social Worker assesses the patient's psychosocial and continuum of care needs.
Interventions may include, but are not limited to: • Family support • Crisis management • Consultations with a staff physician • Adult or Child Protective Services reporting/coordination • Custody/guardianship issue coordination • Indigent patient program coordination • Substance-use support coordination • Intimate partner violence support • Bereavement
Additionally, interventions may include referrals and coordination of acute rehabilitation, extended care, durable medical equipment, home health, and other resources as necessary.

Kentucky - Certified Social Worker (CSW), or Indiana - Licensed Social Worker (LSW)
