Job Title: Social Worker
Location: Turlock, CA 95382
Shift: 7:30 AM 4:00 PM
Payrate: $68-$72/hour
Responsibilities:- Conduct complex psychosocial assessments, reassessments, and interventions
- Assist with adoption, abuse, and neglect cases; provide referrals as needed
- Coordinate patient care and transition planning
- Implement and oversee transition plans
- Lead multidisciplinary care conferences and complex case reviews
- Make referrals and collaborate with physicians, staff, and departments
- Communicate care plans with patients and families
Qualifications:- Social Worker License
- 3 years experience
