Job Description:

We are seeking a compassionate and dedicated Social Worker to join our team in Sterling Heights. The Social Worker will be responsible for providing support and assistance to individuals and families in need. This includes conducting assessments, developing care plans, connecting clients with resources, and advocating for their well-being.

Responsibilities:

• Conduct assessments to determine clients' needs
• Develop individualized care plans
• Provide counseling and support to clients
• Connect clients with community resources and services
• Advocate for clients' rights and well-being
• Collaborate with other professionals, such as healthcare providers and social service agencies
• Maintain accurate and up-to-date client records

Qualifications:

• Bachelor's or Master's degree in Social Work
• Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) preferred
• Experience working with diverse populations
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of community resources and services
