As a Licensed Clinical Social Worker (LCSW), you'll play a key role in supporting patients mental and emotional well-being as part of an integrated care team. This position involves direct patient care, counseling, and coordination with both internal teams and external resources to provide comprehensive support across a variety of medical and behavioral health needs.

What Youll Do
Conduct mental health assessments, brief interventions, and integrated care consultations for patients across all age groups
Provide individual and family counseling tailored to a variety of needs and backgrounds
Educate patients and families on behavioral health topics, treatment options, and available support services
Collaborate closely with medical and behavioral health professionals to ensure coordinated, patient-centered care
Refer patients to appropriate community resources and follow up to support continuity of care
Maintain timely and accurate documentation, including assessments, treatment plans, and progress notes
Participate in team meetings, trainings, and ongoing professional development
Contribute to the development or improvement of behavioral health programs as needed
What Youll Need
Active LCSW license
Experience providing clinical care in a healthcare or integrated care setting
Strong communication and collaboration skills
Familiarity with community mental health and substance abuse resources
Comfort working with diverse populations and age groups
Proficiency with electronic health records and clinical documentation
