Think is in search of a Social Worker who possesses the talents, skills, and knowledge to serve as a vital member on our team. They will need to naturally connect with a wide variety of individuals and develop trusting relationships with the people they serve.

Hours: Monday-Friday 8:00 a.m. to 4:30 p.m.

Location: On-site

Role Summary:

As a Master of Social Work (MSW), you know that quality care is about much more than just addressing physical needs. That's why our Social Worker will take care to understand the social, spiritual and emotional factors related to our patients' health. As a social worker, you will need to develop and maintain community contacts; utilizing available community resources to meet patients individual needs.

Key Responsibilities:

  • Provides medical social services under the direction of a physician. Assists the physician and other members of the care management team in understanding significant social, spiritual, and emotional factors related to the patient's health problems to establish a plan of care that fosters the personal worth, spiritual well-being, and human dignity of each patient
  • Develops working relationships with patients, families and staff for further understanding of significant social and emotional needs underlying patient's health problem
  • Develops and maintains community contacts, utilizing available community resources to meet patient's individual needs.
  • Participates in the development and review of the plan of care for assigned patients
  • Assesses the social, spiritual, and emotional needs/factors to estimate the patient's and family's capacity and potential to cope with the problems of daily living
  • Helps the patient and family to understand, accept and follow medical recommendations and provide services planned to assist the patient and family in achieving the optimum social, spiritual, and health adjustment within their capacity
  • Prepares the patient to deal with changes and the family to support the patient. Observes, records, and reports social, spiritual, and emotional changes
  • Documents all services in the patient's clinical record
  • Participates in ongoing educational programs
  • Assesses, when appropriate, a familys financial situation, taking into consideration the patients prognosis and medical needs, and referring the family to an agency for financial assistance if necessary
  • Other duties as assigned

Skill, Knowledge, and Experience:

  • Masters in Social Work degree (MSW) required
  • 1-2 years experience in a medical setting where the team approach was utilized preferred
  • Experience in Medical Social Work with elderly population preferred.
