What You’ll Do:
Conduct psychosocial assessments and provide counseling to residents and families
Support admissions, transitions, and adjustment to aging or illness
Collaborate with the interdisciplinary team on care planning and documentation (including MDS)
Assist with discharge planning and coordination of community resources
Advocate for residents and provide crisis intervention as needed
Maintain accurate documentation and support financial/insurance processes
Serve as a liaison with families, agencies, and mental health services
What You’ll Need:
You must have a CSW certificate
At least 1 year of interdisciplinary team experience, with long-term care or hospice preferred
Strong communication, critical thinking, and documentation skills
Ability to work in a fast-paced, emotionally sensitive environment
Proficiency with computer systems and maintaining confidentiality
Ability to collaborate effectively with residents, families, and care teams
