Monday–Friday | Day Shift
Join our interdisciplinary care team as a Hospital Social Worker MSW, where your expertise will make a meaningful difference for patients and families during some of life’s most important and vulnerable moments. In this role, you’ll provide compassionate support, advocacy, and resource connection while helping patients navigate the psychosocial factors that impact their health and care journey.
Working across various areas of the hospital, you’ll partner closely with providers, care teams, patients, families, and community resources to support safe transitions, access to care, and individualized care planning. Responsibilities:
- Assess patient needs and safety: Complete psychosocial assessments to identify needs, strengths, preferences, barriers, safety risks, grief concerns, and transition needs.
- Provide crisis support and advocacy: Offer immediate support to patients and families, advocate for confidentiality, informed consent, autonomy, and self-determination, and assist with mandated reporting when needed.
- Coordinate care and transitions: Collaborate with care teams, community resources, facilities, and other professionals to support safe transitions, continuity of care, and patient-centered goals.
- Educate and connect patients/families to resources: Provide education on advance care planning, available services, guardianship/conservatorship, interpreter needs, and community supports.
- Document and contribute to team-based care: Maintain accurate documentation, participate in interdisciplinary meetings, support care planning, and mentor new staff or interns as requested.
Qualifications:
Education: BSW degree in Social Work Required from an accredited school of social work. MSW degree in Social Work preferred.
Experience: Prefer one year clinical experience in health care.
License(s)/Certification(s): Valid LBSW, LMSW, LISW, or similar licensure obtained within 1 year of hire. Successfully complete Person Centered Care course within 1 year of hire. Valid mandatory Reporter course completion by state(s) requirement.
