Job Summary and Qualifications

As a Social Worker in Case Management at our facility you will be joining a team that has\u202fa passion for creating positive patient interactions and a dedication to caring for the multi-faceted wellness of those we serve. In this role you will:

  • Be responsible for the day-to-day direction of social service patient care, including quality care of the patient from admission to discharge.

  • Evaluate the psychosocial needs of patients and family support systems and coordinate appropriate discharge plans for identified patient populations.

  • Act as a liaison between the facility and resources external to the organization.

  • Implement timely discharge plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care.

  • Provide crisis intervention and support.

  • Assist the Case Manager in facilitating patient movement across the continuum of care and identify and track barriers to patient throughput.

  • Establish and maintain effective cooperative working relationships with social service staff, nursing staff, medical staff, and other departments.\u202f

Qualifications:\u202f

  • Master's degree in Social Work.

  • Current State of Utah Certified Social Worker (CSW) license required; Licensed Clinical Social Worker (LCSW) license preferred.

  • 2 or more years of clinical experience preferred.

  • Knowledge of Utilization Review, DRG review, and Discharge Planning preferred.
