Job Summary and Qualifications

* This is a  Full time Day Shift position, hours are 8:00am - 4:30pm and may include weekends as needed *

The Social Worker will assist in the facilitation of care across the continuum for patient care need in the Inpatient Rehab Unit (IRU) with a focus on the following: 

  • Evaluation of psychosocial needs based on physical, emotional, and social barriers and physical limitations
  • Crisis intervention and end of life support
  • Develop and implement weekly care plans the the IRU team and patient/family in goals, treatment, and discharge planning process
  • Ability to collaborate with case managers, physicians, staff, families, and community referral bases to enhance optimal patient outcomes
  • Ability to coordinate service delivery to ensure continuity of care 
  • Ability to recognize age specific, cultural, and religious needs and behaviors of the IRU patientA
  • Ability to problem solve, and seek assistance from appropriate community agencies 
  • Working knowledge of community, regional and governmental resources 
  • Counseling skills 
  • Support the Mission, Vision, and Values of the Organization 
  • Knowledge of federal, state, and local laws, regulations, Joint Commission, CARF and mandates as they relate to social work/case management. 
  • Medicare and Medicaid regulations and payer guidelines related to discharge planning 
  • Other duties as assigned 

 What qualifications you will need:

  • Masters Degree required in Social Work from a school accredited by the Council of Social Work Education
  • Minimum of 1 year experience in an acute care hospital or inpatient rehab facility within the last three years, required
  • Current license by the State of Texas Board of Social Work examiners through the TX department of Health
  • Strong communication skills in both oral and written format
  • Three years post BSW experience preferred
  • Advanced certification by approved accreditation board preferred
