Job Description

The Social Worker IV provides advanced social work and case management services for patients across acute care, long-term care, behavioral health, emergency, home care, and outpatient settings. This experienced clinician manages complex cases, coordinates discharge planning, advocates for patients and families, and collaborates with interdisciplinary teams to ensure compliance with state and federal regulations while promoting safe, patient-centered care.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments, provide counseling, crisis intervention, and manage complex patient cases across multiple care settings.
  • Coordinate discharge planning by collaborating with physicians, interdisciplinary teams, community agencies, and post-acute care providers to ensure safe transitions of care.
  • Educate patients and families on advance directives, healthcare decision-making, insurance resources, financial assistance, and available community services.
  • Advocate for patients by coordinating guardianship, legal proceedings, substance abuse referrals, transportation, facility transfers, and other supportive services as needed.
  • Provide technical guidance to healthcare staff and social workers, ensure compliance with regulatory standards, and participate in quality improvement initiatives.
  • Maintain accurate documentation, participate in interdisciplinary care planning, and perform other related duties to support high-quality patient care.

Minimum Qualifications:

  • Master's degree in Social Work (MSW).
  • One (1) year of specialized social work experience performing complex case management; healthcare social work experience preferred.
  • Demonstrated supervisory aptitude.
  • Valid State of Hawaii driver's license.

Pre-Employment Health Requirements

Offers of employment will be contingent on successfully passing a pre-employment examination, which includes a drug screen and other regulatory medical requirements such as, but not limited to, a tuberculosis (TB) screen.  The cost for drug screening and blood draw for QuantiFERON and Titers, shall be borne by the Hawaii Health Systems Corporation.
