Job Description

Social Workers seek to improve the overall quality of life of people within the context of their environment through the remediation and prevention of their problems, and by improving accessibility, accountability and coordination of service delivery among a variety of professionals and/or agencies tasked with providing appropriate services to patients.  Social Workers establish working relationships of mutual respect and trust with the patients and families served, and utilize a variety of helping interventions to enable these patients to function as effectively as possible.

This position will be cover patients in Transitional Care Unit & Behavioral Health Unit.

Key Responsibilities:

  • Provides comprehensive social services (assessment, counseling, intervention, and advocacy) to patients and residents, including psychosocial evaluations, reassessments, and psychiatric assessments within required timeframes.
  • Offers guidance on financial and insurance matters, makes referrals, and educates patients and families on advance care planning (e.g., advance directives, power of attorney, living wills).
  • Acts as a patient advocate and may initiate legal processes such as guardianships while ensuring compliance with regulatory requirements.
  • Reviews and oversees social service documentation, may supervise or sign off on other social workers’ notes, and can serve in a rotating leadership/coordination role.
  • Collaborates with interdisciplinary teams to determine level of care and coordinate treatment and discharge planning.
  • Facilitates safe and appropriate discharges by arranging transportation, transfers, community services, and referrals (including substance abuse treatment).
  • Supports continuity of care by coordinating post-hospital services, documenting care provided, and engaging legal or court resources when needed to protect patient interests.

Minimum Qualifications

Education & Experience:

  • Education:
    • Graduation with a bachelor’s degree or master’s degree in social work from a program of study accredited by the Council on Social Work Education, or a doctoral degree in social work from a college or university accredited by the Western Association of Schools and Colleges, or a comparable regional accreditation body.

Pre-Employment Health Requirements

Offers of employment will be contingent on successfully passing a pre-employment examination, which includes a drug screen and other regulatory medical requirements such as, but not limited to, a tuberculosis (TB) screen.  The cost for drug screening and blood draw for QuantiFERON and Titers, shall be borne by the Hawaii Health Systems Corporation.
