Position Overview
This position is located in Adult Services.
Examples of Duties
This position is within the General Services Unit. The workload consists of Special Assistance In Home (SA-IH) cases. The worker in this position assesses the needs of individuals in the community and provides intensive and comprehensive case management for customers with deteriorating social functioning. This includes development of realistic and obtainable goals as well as a specific work plan. The worker must observe and identify the health care needs of customers and access a broad scope of services. This position requires the worker to possess an extensive knowledge of many specialized and supportive services programs to enhance prospects for elderly/disabled individuals to remain in their home. A major component of this position is the role of advocate for the customer.
Minimum Qualifications
MINIMUM EDUCATION AND EXPERIENCE: Bachelors degree in social work from an appropriately accredited institution; bachelors degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelors degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience. Directly Related Experience is defined as Human Services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.
Knowledge, Skills and Abilities
KNOWLEDGE, SKILLS AND ABILITIES: Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group work and community problems. Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment. Knowledge of governmental and private organizations and community resources. Knowledge of laws, regulations and policies which govern the program. General knowledge of medical terminology, disease processes and their treatment is required In certain programs or settings. Skill in establishing rapport with a client and applying techniques of assessing psychosocial. behavioral, and psychological aspects of clients problem. Ability to establish and maintain effective working relationships with administrative superiors members of case load and their families, and with civics medical, social and religious organizations. Ability to train or instruct lower-level social workers, students, or interns. Ability to express Ideas clearly and concisely and to plan and execute work.
Knowledge of Geriatrics, the Older American Act, the Developmentally Disabled Institute, Preventive Intervention Techniques, and Protective Services. Knowledge of casework and community services and service programs. Good verbal and written communication skills are essential to the position. The ability to comprehend, interpret, and apply the regulations which govern Adult Care Home (ACH) licensure is needed. Also the ability to effectively articulate issues related to licensure to North Carolina Division of Facility Services consultants, ACH personnel, residents and families and other community groups/organizations is required. This position must be able to comprehend, interpret and apply the regulations and policy related to the Special Assistance - In-Home section in the Adult Services Manuel and the At-Risk Adult section of the Division of Medical Assistance Manual.

The employee in this position will be operating motor vehicles for Cumberland County and shall have a valid North Carolina Drivers License.

Law enforcement background check is required.
Duties of this position require the use of the employees personal vehicle.
This position will require assisting in emergency and/or disaster situations.
