Are you a compassionate and experienced Social Worker looking to make a meaningful impact—while enjoying the flexibility and support of a collaborative team? At Care New England, we’re looking for dedicated professionals to join our VNA team and help patients and families navigate life’s most important healthcare moments right at home.


What You’ll Do:

As a key member of our interdisciplinary care team, you’ll support patients and families by providing emotional guidance, resource coordination, and advocacy in the hospice settings. You’ll collaborate closely with nurses, therapists, and physicians to ensure comprehensive care and a strong support system for every patient.


Your Day-to-Day Might Include:

• Offering emotional and psychosocial support to patients and families
• Helping patients and caregivers set healthy boundaries and goals
• Identifying high-risk situations and coordinating timely interventions
• Referring families to community services and financial assistance programs
• Supporting hospice patients with bereavement and respite care planning
• Participating in team case conferences and care planning
• Providing supervision to other social work staff (if licensed at LICSW level)
• Documenting assessments, plans, and outcomes in a timely and accurate manner

What You’ll Need:

• Bachelor’s Degree in Social Work (BSW) required
• Master’s Degree in Social Work (MSW) preferred
• LCSW or LICSW preferred
• 3–5 years of social work experience (hospice a plus)
• Valid driver’s license and auto insurance
• Strong communication, problem-solving, and advocacy skills
