South East Chicago City with focus on 60617, 60619, 60620, 60628 AND 60643 communities. Weekdays 8-5. M-F. Weekend and holiday rotations.

Pay Range

$30.70 - $46.05

The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required. Assesses the home care patient's psycho-social needs as related to their family background and environment as it impacts the medical condition and the prognosis for positive outcome; formulate and implement an effective treatment program to meet those needs; and coordinate with all disciplines regarding the patient's social welfare.

• Patient Relationship: Develops and maintains a therapeutic relationship throughout the healthcare continuum.
• Assuring and Improving Quality Care: Facilitates the patient and family's right to receive quality cost effective care following Advocate's MVP of Stewardship.
• Patient Education: Demonstrates a commitment to meet the learning needs of patients and their families. Application of a systematic process involving optimal level of self care for patient and optimal quality of life for patient, family and significant other following Advocate's MVP of Equality.
• Care and Service Coordination: Participates as an effective member of the patient care team to formulate an integrated approach to care, following Advocate's MVP of Partnership.
• Leadership/Collaborative/Professional Development: Demonstrates respect and understanding of peers and other clinical disciplines and participates as an effective member of the patient care team to formulate an integrated approach to care following Advocate's MVP of Excellence
• Business Development and Growth: Demonstrates behaviors that enhance productivity and an understanding of financial and economic conditions in the health care environment.

Education/Experience Required:

• Masters of Social Work services from an accredited school with licensure or licensed social worker (LSW) or licensed clinical social worker (LCSW). 1-2 years of experience in a clinical setting post licensure preferred.
• A valid driver's license issued by the Division of Motor Vehicles

Knowledge, Skills & Abilities Required:

• Basic knowledge of computer use (i.e. Microsoft Word, email access and use) Ability to learn and use computer based scheduling, and documentation system Ability to communicate professionally both verbally and through written reports with strong documentation skills Good interpersonal skills Good time management and self organization skills
• Licensure in Illinois.

This position is eligible for a $5000.00 sign on bonus! You must be employed in the exact position at time of payout. Internal team members are not eligible for this sign on bonus at this time.

Physical Requirements and Working Conditions:

• Extensive travel within geographical boundaries in all extremes of weather and on all types of roads including expressways. Must be able to provide/arrange own transportation to and from patients homes and branch office. Ability to work rotating schedules as needed. Requires adherence to scheduled days and hours of work based on commitment to patient/client and work load assigned.
• If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
