Full-Time Social Worker | Monday-Friday, 8:30 AM-5:00 PM
Are you passionate about supporting others and making meaningful connections? Glacier Hills is seeking a dedicated Full-Time Social Worker to join our Social Services Department and play a vital role in enhancing the lives of our residents and their families.
About the Role
As a Social Worker at Glacier Hills, you will be an advocate, resource, and trusted partner in care. Your responsibilities will include:
Participating in discharge planning, resident assessments, and the development and implementation of individualized social care plans
Conducting cognitive and mood assessments and coordinating appropriate follow-up care and treatment
Completing timely and accurate documentation, including medical forms, reports, evaluations, and charting
Referring residents and families to community resources and appropriate social service agencies
Educating residents and families on Medicare, Medicaid, and other financial assistance programs
About You
You are compassionate, organized, and committed to resident-centered care. Qualified candidates will have:
Active Social Worker licensure in the state of Michigan
A Bachelor's or Master's degree in Social Work or a related human services field (such as Sociology, Psychology, Special Education, or Rehabilitation Counseling)
One year of supervised social work experience in a healthcare setting (preferred)
Experience working in long-term care or with the geriatric population (preferred)
