Salary range: $54,860.00 - $80,232.75 per year

How you'll make an impact in this role:

 

  • Conduct interviews and assessments to establish participant priorities and ensure accurate case documentation.
  • Coordinate treatment plans with interdisciplinary teams, agencies, and caregivers to ensure compliance.
  • Facilitate referrals to community resources and support programs for participants and caregivers.
  • Build strong community partnerships to address participants' evolving health and social needs.
  • Manage concurrent reviews and discharge planning, coordinating social services and post-discharge support.
What minimum qualifications you'll need:

Licensure / Certification / Registration:

 

Preferred Credential(s):

  • Drivers License.
  • Masters Level Social Worker.

Education:

  • Master's degree in Social Work required.

Work Experience:

  • 3 years of experience preferred.
What additional requirements you'll need:
  • 1-2 years of experience in case management, home care and/or clinical therapy required.
  • Limited or full licensure in the State of Michigan is required.
