Pay Range: $47.57 - $58.80 per hour depending upon experience

Job Summary:


The Social Worker is responsible for providing a variety of direct and related services to patients, their families, and the professional staff. Incumbent is guided by a thorough knowledge of the psychosocial evaluation process, social work intervention, rules & regulations of the Dept. of Health, Education, Welfare, and the rules & regulations of the hospital.


Responsibilities:

  • Completes psychosocial evaluations
  • Completes collaborative treatment planning and implements patient centered services, involving family and /or care partners in the treatment plan process
  • Conducts individual, group psychotherapy as prescribed by the treatment plan
  • Participates in departmental meetings
  • Documents according to clinical standards
  • Develops after care plans for patients upon discharge including linkages to other agencies as appropriate
  • Proficiency in implementing Suicide Risk Assessment
  • Proficiency in implementing Telehealth Services Policy in compliance with Federal and
  • NYS regulatory guidelines

Required Skills:

  • Master of Social Work
  • Valid NYS LCSW or LMSW
  • Minimum 3-5 years of experience in Behavioral Health Outpatient setting
  • Bilingual English/Spanish preferred
  • Must be computer literate, familiar with Microsoft Word & Excel
  • Must be able to problem solve and think critically
