Description

Job Title: Licensed Clinical Social Worker (LCSW) – Hospital-Based

Duration: 12 Weeks
Location: 1000 Medical Center Boulevard, Lawrenceville, GA 30046
Schedule: Nights | 11:00 PM – 7:30 AM | Monday–Friday | 5x8-Hour Shifts | 40 Hours/Week

Job Requirements

  • Minimum 2 years of Social Work experience within an acute care hospital setting.
  • Active Georgia Licensed Clinical Social Worker (LCSW) license required.
  • Recent acute hospital experience required.
  • Strong knowledge of psychosocial assessments, crisis intervention, discharge planning, and community resources.
  • Excellent communication, interdisciplinary collaboration, and documentation skills.
  • No local candidates within 50 miles of the facility.

Behavioral Health Services (BHS) – Hospital-Based Social Worker

Roles & Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patients' emotional, social, financial, and discharge planning needs.
  • Develop and implement individualized care and discharge plans in collaboration with physicians, nurses, case managers, and the interdisciplinary care team.
  • Provide crisis intervention, emotional support, counseling, and advocacy for patients and their families.
  • Coordinate referrals to rehabilitation facilities, skilled nursing facilities, hospice, home health agencies, behavioral health programs, and community support services.
  • Assist patients and families with advance directives, guardianship issues, and end-of-life planning when appropriate.
  • Assess patients for abuse, neglect, domestic violence, substance use, mental health concerns, and other psychosocial risk factors, ensuring compliance with mandatory reporting requirements.
  • Educate patients and families regarding available healthcare resources, insurance coverage, financial assistance, and community programs.
