This position is located in Community Long Term Care (CLTC), Spartanburg County.

This is an in-office role and not a telecommute or remote position.

Are you the One? We are looking for a Social Worker (Case Manager II), who works under general supervision of CLTC Area Administrator. Serves as Case Manager Il. Assists in the implementation of a system of case management for Medicaid eligible applicants and enrolled participants. Assists other CLTC offices with essential job functions according to program needs. Trains workers and performs assigned quality assurance activities and compliance activities as directed. Travel to location of assigned cases necessary for this position. Maintains a thorough working knowledge of all aspects of CLTC and related systems: programs, policies, and procedures, including but not limited to waivers, Phoenix and Care Call.

  • Completes waiver enrollment activities.
  • Provides case review related to assessment and service planning.
  • Conducts service Prior Approval process.
  • Performs Quality Assurance and Compliance Activities.
  • Provides case management services.
  • Promotes Collaboration and Effective Communication.
  • Performs emergency preparedness duties or activities as required.

Minimum and Additional Requirements

A Bachelors Degree in social work or social welfare program accredited by the Council on Social Work Education, or from a social work or social welfare program offered by a regionally accredited institution, plus one (1) year of experience as a professional social worker.

MANDATORY SPECIAL REQUIREMENT: Must have current licensure by the SC Board of Social Work Examiners.

Preferred Qualifications

  • The ability to multi-task within time frames and prioritize cases utilizing effective time management skills.
  • Effective time management skills and computer proficiency.
  • Must be able to perform web-based software functions (key participant data, run reports, update documents, etc.).
  • Knowledge of criteria for level of care determination.
  • Knowledge of federal and state Medicaid regulations.
  • Understanding and interest in working with the elderly and disabled.
  • Ability to utilize and accept supervision and consultation.
  • Ability to train new staff and contract providers.
  • Ability to communicate effectively both verbally and in writing.
  • Personal computer skills.
  • Basic math skills.
  • Ability to make and keep schedules.
  • Community Long Term Care experience and gerontology preparation.
