Your future role at a glance:

Location: Jacksonville, FL 

Facility: Ascension St. Vincent's Southside Hospital 

Schedule: Day Shift | Part time

How you'll make an impact in this role:
  • Coordinate the interdisciplinary plan of care from admission to discharge.

  • Create care plans that integrate patient preferences and family values.

  • Monitor patient progress through consistent assessments and record evaluations.

  • Advocate for essential resources and proactively remove barriers to care.

  • Act as a clinical resource for medical documentation and post-discharge options.

What minimum qualifications you'll need:

Licensure / Certification / Registration:

  • Clinical Counselor preferred.
  • Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
