Salary range: $40.00 - $42.00 per hour
How you'll make an impact in this role:Coordinate the overall interdisciplinary plan of care from admission to discharge, integrating patient and family preferences, while requiring an LSW or LCSW to manage the process.
Monitor patient care through assessments, evaluations, and medical records while acting as a resource for documentation accuracy and treatment appropriateness.
Advocate for necessary resources, remove barriers, and coordinate options for post-discharge care.
Maintain ongoing dialogue with supervisors and other healthcare providers to ensure effective implementation of the health plan.
