Social Worker Care Management | Full-time, 80 hours bi-weekly
Monday-Friday, 8am-4:30pm/12p-8pm
Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge. Key Responsibilities:
Coordinate psycho-social support for patients during hospitalization
Develop and implement personalized discharge plans
Assess patient needs related to care coordination, resources, and services
Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction
Facilitate access to community resources to support recovery and continuity of care
Ensure compliance with hospital policies, regulatory standards, and evidence-based practices
Qualifications: Education:
Bachelor's degree in Social Work required
Master's degree preferred
Experience:
Licensure/Certification:
LBSW license in Maryland required
Must obtain care/case management certification within five years of hire
Skills & Abilities:
Strong communication and multitasking skills
Familiarity with EMRs and ability to navigate electronic medical records
Knowledge of disease processes, regulatory requirements, and evidence-based care
Understanding of community resources, provider networks, and the revenue cycle
Ability to build relationships across clinical and community settings
