    Social Worker Care Management | Full-time, 80 hours bi-weekly

    Monday-Friday, 12p-8pm

    Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge. Ideal candidates thrive in interdisciplinary environments and are committed to improving patient outcomes through holistic, evidence-based care.

    Key Responsibilities:

    • Coordinate psycho-social support for patients during hospitalization

    • Develop and implement personalized discharge plans

    • Assess patient needs related to care coordination, resources, and services

    • Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction

    • Facilitate access to community resources to support recovery and continuity of care

    • Ensure compliance with hospital policies, regulatory standards, and evidence-based practices

    Qualifications: Education:

    • Bachelor's degree in Social Work required

    • Master's degree preferred

    Experience:

    • Minimum two years in acute care, SNF, physician offices, or community health settings

    Licensure/Certification:

    • LBSW license in Maryland required

    • Must obtain care/case management certification within five years of hire

    Skills & Abilities:

    • Strong communication and multitasking skills

    • Familiarity with EMRs and ability to navigate electronic medical records

    • Knowledge of disease processes, regulatory requirements, and evidence-based care

    • Understanding of community resources, provider networks, and the revenue cycle

    • Ability to build relationships across clinical and community settings
