Overview

Responsibilities

Plans, organizes, coordinates, and implements comprehensive social work services to patients in the setting to which they are assigned to ensure appropriate and timely discharge planning and provision of services including the social, emotional, economic and environmental needs that may affect the patient's medical condition and/or progress. Develops and implements plans of care based on identified needs.

Qualifications

Minimum Qualifications:

• Bachelor's degree in Social Work

Licensure/Certification/Registration:

• Licensed in Alabama as a Licensed Bachelor Social Worker (LBSW)
