Under the supervision of a master prepared social worker, provides counseling to families, individuals and groups. Works in collaboration with multidisciplinary teams, coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff.

Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.
Interview patients, assess priorities and document case activity.
Consult with team managers, statutory and voluntary agencies and patient''s relatives to ensure compliance with patient''s treatment plan.
Assist clients/families with obtaining community assistance by referral to proper resources.
Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.
Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
Remain current with all licensure, certifications and mandatory compliances and trainings required of this position.
Adhere to all policies, procedures and practices (Regents Rules, TTUS, HSECEP OPs, etc.).
