Job Summary:
To attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident/patient and address associated family es. To access eligibility for Medicare, Maine Care, or managed care benefits linked to community resources.
Essential Duties and Responsibilities
- Maintain regular contact with residents/patients of assigned units.
- Provide emotional support to residents/patients and families as needed.
- Assist residents/patients in adjusting to new environments.
- Conduct family conferences and facilitate support groups.
- Support and reinforce coping skills of residents/patients and families.
- Address complaints and concerns raised by residents/patients, families, and staff.
- Assist residents/patients and families with financial planning and MaineCare applications.
- Provide referrals to community agencies to meet residents''/patients'' needs.
- Interpret resident rights issues and advocate for victims of abuse or neglect.
- Report incidents of abuse as mandated by law.
- Facilitate Advance Directives discussions.
- Collaborate with interdisciplinary teams in planning resident/patient care.
- Maintain current records on residents/patients, including social history and discharge plans.
- Ensure confidentiality of medical records and Social Service information.
- Coordinate intra-facility transfers and moves with nursing staff.
- Present a positive image of the facility to potential residents/patients and families.
- Foster positive relationships with community social service agencies.
- Stay updated on Federal, State, Medicare, MaineCare, and managed care, regulations.
- Attend departmental meetings and engage in professional growth activities.
- Formulate discharge plans with residents/patients, families, and interdisciplinary team members.
- Provide referrals to community social service agencies for continued care.
- Collaborate with attending physicians and adhere to State and Federal regulations for discharge arrangements.
- Make recommendations for improvements in quality and efficiency.
- Maintain safety standards for residents/patients, guests, and staff.
- Perform additional duties as requested by immediate supervisor.
- Supports and promotes the mission and values of Covenant Health Ministry.
- Other duties as consistent with this role.
Job Requirements
Job Knowledge and Skills
- Proficiency in computer applications and electronic medical record systems.
- Excellent organizational and multitasking abilities with strong attention to detail.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Demonstrate customer service skills and a friendly, professional demeanor.
- Flexibility to adapt to changing priorities and work schedules.
- Knowledge of medical terminology and procedures is desirable.
- Must be able to speak, read, write, and comprehend the English language.
Education and Experience
- Bachelor's degree in social work or similar field
- Valid State of Maine Licensed Social Worker (LSW) license required.
- 0-1 year of Experience preferred in social work, preferably in a healthcare or nursing home setting.
